4. Health Information and Medical Services

General information on our website, social media, and promotional communications is provided for educational purposes. It is not individualized medical advice, diagnosis, or treatment.

Browsing our website, submitting a general inquiry, or receiving promotional communications does not by itself establish a provider-patient relationship. Clinical care is subject to appropriate evaluation, acceptance as a patient, and applicable consent and service agreements.

Do not disregard professional medical advice or delay seeking care because of information found on our website. Individual results vary, and no specific treatment outcome is guaranteed.

Our website, email, and text messaging services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or seek immediate emergency care.

5. Payments and Refunds

Applicable prices, payment terms, deposits, and refund conditions will be disclosed before you purchase or schedule a service.

You agree to pay charges you authorize according to the payment terms provided to you. Any recurring billing arrangement requires a separate disclosure and your authorization.

Refund eligibility is governed by the policy provided at the time of purchase and applicable law. For billing or refund questions, contact [email protected] or (281) 336-9030.

6. Scheduling and Cancellations

Appointments are subject to availability and confirmation.

Please contact us promptly if you need to cancel or reschedule. Any cancellation deadline, missed appointment fee, late arrival policy, or deposit requirement will be disclosed when you book or in a separate service agreement.

Impersonate another person or submit false information.

Access another person’s information without authorization.

Share account credentials or permit unauthorized account access.

Interfere with website operation or security.

Upload malicious software or use the website for fraudulent or unlawful activity.

Our SMS program provides text messages for appointment reminders, events, receipts, customer service, and occasional promotional messages when you opt in. Message frequency varies depending on your interaction with our services.

8.1 Opting Out

You can cancel the SMS service at any time. Simply reply STOP to the number sending you messages. After receiving your STOP request, we will send a text confirming that you have unsubscribed. Following this confirmation, you will no longer receive messages from our SMS program.

8.4 Message & Data Rates

Message and data rates may apply to messages sent to you by us and to messages you send to us. Message frequency varies. For questions about your text or data plan, please contact your wireless provider.

8.5 Recurring Message Disclosure

By opting into our SMS program, you authorize Longevity Health, LLC to send recurring text messages to the mobile number you provide. Depending on your service selection, you may receive regular recurring messages.

You are not required to agree to receive recurring promotional messages as a condition of purchasing any goods or services.

8.6 Prohibited Content

Our SMS messages will never contain phishing attempts, smishing, or social engineering intended to manipulate you into sharing private information.

Our messages will not contain illegal content or prohibited SHAFT content—sex, hate, alcohol, firearms, and tobacco—in violation of applicable laws or messaging provider requirements.

8.7 SMS Opt-In Data Protection / Privacy for SMS Services

Text messaging originator opt-in data and consent information will not be shared with third parties, except aggregators and providers of text messaging services as necessary to deliver the SMS service.

9. Privacy

Our Privacy Policy explains how we collect, use, and disclose personal information.

Patient medical information may also be governed by our separate Notice of Privacy Practices and applicable health privacy laws. These Terms do not replace any required patient authorization or informed consent.

10. Third-Party Services and Links

Our website may include links to third-party websites or tools, including scheduling platforms, patient portals, or payment services.

These services may have separate terms and privacy policies. A link does not by itself constitute our endorsement of the third party’s content or practices.

11. Website Availability and Accuracy

We make reasonable efforts to keep website information accurate and current, but errors or omissions may occur. We may correct content and update service descriptions as needed.

We do not guarantee uninterrupted or error-free website access. Website maintenance, technical issues, or circumstances outside our control may affect availability.

12. Website Disclaimer

To the extent permitted by applicable law, our general website and educational materials are provided on an “as is” and “as available” basis, without warranties concerning uninterrupted availability, completeness, or fitness for a particular purpose.

This disclaimer applies to general website use and educational content. It does not alter professional duties associated with clinical care or any rights that cannot lawfully be excluded.

To the extent permitted by applicable law, Longevity Health, LLC will not be liable for indirect, incidental, special, or consequential damages arising from your use of, or inability to access, our general website or educational materials.

Nothing in these Terms excludes or limits liability that cannot lawfully be excluded or limited, or waives applicable patient or consumer rights.

14. Governing Law

These Terms are governed by the laws of the State of Texas, together with applicable federal law, without overriding any mandatory protections available under applicable law.

15. Changes to These Terms

We may update these Terms as our services, practices, or legal obligations change. Revised Terms will be posted with an updated effective date.
